Además, el nivel de expresión de MDR1 está regulado por el receptor nuclear PXR. En el presente trabajo de Tesis Doctoral evaluamos la influencia de los polimorfismos de nucleótido simple (SNP) de MDR1 (T-129C, C1236T y C3435T) y PXR (C63396T) en la infección pediátrica por HIV-1, la respuesta al tratamiento antirretroviral y la expresión de P-gp. Los SNPs C1236T y C3435T cumplirían un rol protector retrasando la progresión a SIDA pediátrico, pero no afectarían la transmisión vertical por HIV-1. Además, evaluamos el efecto de los SNPs en la respuesta antirretroviral al PI lopinavir, demostrando que el SNP C1236T se asocia con una menor concentración plasmática de la droga y una mayor carga viral. Por otra parte, analizamos si los niveles de expresión de P-gp en linfocitos CD4+ se asocian con los SNPs de MDR1 o PXR, sin hallar diferencias entre los distintos genotipos. Sin embargo, observamos que el nivel de expresión de P-gp en linfocitos CD4+ aumenta con la edad y la infección pediátrica por HIV-1. En conjunto, nuestros resultados sugieren que P-gp es un factor clave tanto en la modulación de la infección por HIV-1, independientemente de su función en el transporte de drogas, como en la respuesta a la terapia antirretroviral. The multidrug resistant gene (MDR1) encodes for P-glycoprotein (P-gp), which pumps several substrates out of the cell, such as the antiretroviral protease inhibitors (PI). In addition, the MDR1 expression level is regulated by the nuclear receptor PXR. In the present PhD Thesis we evaluated the influence of the single nucleotide polymorphisms (SNP) T-129C, C1236T y C3435T of MDR1 and C63396T of PXR in HIV-1 pediatric infection, response to antiretroviral therapy and P-gp expression. The SNPs C1236T and C3435T may play a protective role delaying progression to pediatric AIDS, but we did not observe an effect HIV-1 vertical transmission. In addition, we evaluated the effect of SNPs on antiretroviral response to the PI lopinavir, demonstrating that the SNP C1236T is associated with a lower drug plasma concentration and a higher viral load. We analyzed whether P-gp expression levels in CD4+ lymphocytes are associated with the SNPs of MDR1 or PXR, but no differences between the genotypes were found. However, we observed that P-gp expression levels in CD4+ lymphocytes increase with the age and the HIV-1 pediatric infection. Our results suggest that P-gp is a key factor either in the modulation of the HIV-1 infection, independently of its function in drug transport, or in the response to antiretroviral therapy.
