The majority of patients with elongated styloid are asyntomatic. The syndrome is diagnosed based on clinical presentation and with imaginologic confirmation. The styloid is considerated elongated after 30 mm lenght. Little or none correlation between lenght and symptoms.Originally described as facial and tonsilar pain post— tonsilectomy. Since then appeared reports of others pathologies associated with elongated Styloid process like, dental pain, carotid disections, internal jugular vein stenosis, Horner sign, sudden death, carotid artery aneurrisms, third brach trigeminal neuralgia, lingual hemiatrophy and fasciculations and so on. We discuss imaginologic diagnosis and treatments, specially surgery and emphasize the scarce alert to include this pathology among the diagnosis to be considered in the anatomical area. El síndrome de Eagle es una patología poco frecuente causada por un proceso estilohioideo elongado osificado. La estiloides elongada es habitualmente asintomática. Se diagnostica por su presentación clínica y se confirma por imágenes. La estiloides está elongada cuando mide más de 30 mm. Pero sin correlación entre el largo con la intensidad de la clínica. La correlación entre el largo de la estiloides y los síntomas es escasa o nula. El síndrome fue descripto originalmente como dolor tonsilar/facial post amigdalectomía. Desde entonces aparecieron comunicaciones de otras patologías asociadas a la elongación del proceso estiloideo como dolor dental, disección carotídea, estenosis de la vena yugular interna, signo de Horner aislado o acompañado, muerte súbita, aneurismas carotídeos, neuralgia trigeminal de 3ª rama, hemiatrofia lingual y fasciculaciones y otros. Discutimos los diagnósticos imagenológicos y tratamientos, especialmente quirúrgicos y enfatizamos el escaso alerta para incluir esta patología entre los diagnósticos zonales.
