Based on the trajectory of different authors a theorization of the theme is attempted, especially considering metaphysics aspects that allow to establish the difference between the night-mare and the dream. Taking up a constellation of concepts from the navel of the dream to the sinister and pain, a reading horizon is created and both theoretical and clinical aspects are taken into account. Nuestro trabajo se organiza en un doble sentido: en primer lugar, introducimos la cuestión de los Cuidados Paliativos al psicoanálisis; en el segundo, y en un sentido inverso, introducimos el psicoanálisis a los Cuidados Paliativos. Justificamos este movimiento en el sostener que el psicoanálisis tiene qué decir y qué ofrecer en cuidados Paliativos. Siendo que en ambos terrenos encontramos como común denominador el concepto del dolor, respondemos a la pregunta acerca del qué tiene el psicoanálisis para decir en Cuidados Paliativos mediante la lectura particular que puede ofrecer de tal fenómeno. Pero llegando a este punto, decimos que lo más importante no es lo que tenga el psicoanálisis para decir, sino lo que tenga por escuchar, en la oferta que hace de la apertura de un espacio para que se desplieguen los significantes de un sujeto.
