description Objective: The aim of this work was to study clinical features of HPV-associated Oral Epithelial Dysplasia (HPV-OED) and evaluate risk factors for this entity. Methods: There were retrieved clinical records of the Oral Pathology Department – Facultad de Odontología, Universidad Nacional de Córdoba with histopathological diagnostic of HPV-OED. Age, sex, site, tobacco and alcohol habit, chief complaint, clinical presentation, mechanical irritation associated, status of adjacent mucosa, presence of histopathologic koilocitosis, degree of epithelial dysplasia and PCR-HPV genotype were studied. Results: We found 11 cases that satisfied the diagnostic criteria for HPV-OED. HPV-OED was presented predominantly in females with a average age of 61 years old. The lateral border of the tongue was the most frequent site of presentation. Tobacco consumption was present in almost all cases. Painless, whitish plaques with a rough surface were the most common clinical presentation. Chronic mechanical irritation (CMI) was observed in nine cases, only two lesions didn´t have association with CMI. Koylocitosis was found in all cases as the most frequent histological feature. HPV-16 was found in 4/11 cases, being the most frequent genotype. Conclusion: Our findings suggest that HPV-OED could have distinctive features from that present in the genital mucosa. There are other risk factors for Oral HPV infection, not present in other mucosae, which should be study due to its influence in the viral cycle such as CMI. Also, further investigations are needed to address the HPV-OED prognosis.

Resumen: El objetivo de este trabajo es estudiar las características clínico-patológicas de la Displasia Epitelial Oral asociada al VPH (DEO-HPV) y evaluar los factores de riesgo asociados a esta entidad. Materiales y Métodos: se analizaron historias clínicas de pacientes que concurrieron a la Cátedra de Estomatología “A” de la Facultad de Odontología de la Universidad Nacional de Córdoba con diagnóstico histopatológico de DEO-HPV. Del análisis de las historias clínicas, se estudiaron las siguientes variables: edad, sexo, localización de la lesión, consumo de tabaco y alcohol, motivo de la consulta, presentación clínica, presencia de irritación mecánica crónica oral (IMCO), estado de la mucosa adyacente, presencia de coilocitosis, grado de displasia (en el examen histopatológico), y genotipo de VPH detectado (por PCR). Resultados: Se encontraron once casos de DEO-HPV diagnosticados con el triple criterio. La DEO-HPV se presentó predominantemente en el sexo femenino con un promedio de 61 años. El borde de lengua fue el sitio más frecuente de presentación de la lesión. El consumo de tabaco se presentó en casi todos los casos. Las lesiones se presentaron en su mayoría en forma indolora, y con un aspecto clínico de placa blanca, y en algunos casos con tendencia a la verrugosidad. La mucosa periférica a la lesión en pocos casos se encontraba sana. En casi todos los casos presentados, las lesiones estuvieron asociadas a un agente irritativo crónico. Los signos histológicos de infección por VPH fueron informados en todos los casos y el genotipo de VPH más frecuente fue el 16. Conclusión: La presentación de la DEO-HPV tiene características propias, ya que el microambiente bucal genera condiciones específicas que  difieren a la patología ubicada en la mucosa genital. Se deben realizar más trabajos que aborden factores de riesgo y pronóstico de la DEO-HPV.
