This study aimed to analyze the phylogenetic relatedness of HAV strains in Córdoba, Argentina to gain a better understanding of their circulation dynamics. Between 2016-2023, 78 and 109 RNA-HAV+ clinical and wastewater samples, respectively, were collected from Córdoba, Argentina, characterized by 2 RT-Nested-PCR targeting VP1/2A region and were sequenced (Sanger). Two datasets with genotype HAV-IA sequences were built using two length fragments to perform phylogenetic analyzes and compare them: Dataset-A with 531 sequences (300bp) and Dataset-B with 119 sequences (900bp). Both included isolates obtained in this study (n=47); all the previously published sequences from Argentina and Latin-American countries; the first five sequences retrieved from BLAST search with sequences from this research; and reference IA sequences. Phylogeny was performed with IQ-TREE-v2.1. Results from both datasets were similar and revealed that Argentine HAV isolates formed two main clusters. The larger cluster contained sequences from European and Latin American outbreaks among men who have sex with men (MSM), and Argentine isolates from Córdoba (from a local MSM outbreak during 2017-2019; from clinical cases that occurred during 2022-2023, and from wastewater samples from the same years). Within this clade, strains from Córdoba clustered into three groups based on different periods (2017-2018; 2018-2019; 2022-2023) indicating that the outbreak strain has been established in the region since 2017 with small yearly variations. The second cluster included sequences from this study and previous endemic Argentine strains from different regions, sources, and years. The rest of the new sequences obtained were intermingled with other worldwide sequences and corresponded to imported strains from travelers to Latin American countries. Our findings highlight the diversity of HAV genotype IA strains circulating in Argentina and suggest the introduction of genetically diverse strains from geographically distant regions and their potential spread to the local population. These results emphasize the importance of phylogenetic approaches to monitor HAV circulation dynamics, trace the origin of outbreaks, and support evidence-based public health interventions.

En Argentina, la introducción de la vacuna generó cambios en la epidemiología del virus de la hepatitis A (HAV), destacando la necesidad de monitorear las cepas virales circulantes. Este estudio plantea analizar las relaciones filogenéticas de las cepas de HAV en Córdoba-Argentina para una mejor comprensión de su dinámica de circulación. Se analizaron 78 muestras clínicas y 109 de aguas residuales RNA-HAV+, colectadas en Córdoba-Argentina entre 2016-2023, por dos RT-Nested-PCRs dirigidas a la región VP1/2A y posterior secuenciación (Sanger). Se construyeron dos bases de datos (BD) con secuencias de HAV genotipo IA de los dos fragmentos amplificados: BD-A con 531 secuencias (300pb) y BD-B con 119 secuencias (900pb). Ambos incluyeron las secuencias obtenidas en este estudio (n=47); todos los aislamientos de Argentina y Latinoamérica publicados anteriormente; las primeras cinco secuencias obtenidas de la búsqueda de BLAST con secuencias de este estudio; y secuencias IA de referencia. Los análisis filogenéticos se realizaron con IQ-TREE-v2.1. Los resultados de ambas BD fueron similares, revelando que los aislamientos de HAV formaron dos agrupamientos principales. El grupo más grande contiene secuencias de brotes europeos y latinoamericanos en hombres que tienen sexo con hombres (HSH) y aislamientos de Córdoba (del brote local de HSH de 2017-2019, de casos ocurridos en 2022-2023 y de aguas residuales de los mismos años). Dentro de este clado, las secuencias de Córdoba se agruparon en tres grupos según diferentes períodos (2017-2018; 2018-2019; 2022-2023), lo que indica que la cepa responsable del  brote se habría establecido en la región desde 2017 con pequeñas variaciones anuales. El segundo clado incluyó secuencias de este estudio y cepas previas endémicas argentinas de diferentes regiones, fuentes y años. El resto de las secuencias obtenidas agruparon entremezcladas con secuencias globales y correspondieron a cepas importadas de viajeros a países latinoamericanos. Nuestros hallazgos resaltan la diversidad de cepas HAV-IA en Argentina, sugiriendo la introducción de nuevas variantes de regiones geográficamente distantes y su potencial propagación a la población local. Se enfatiza la importancia de los enfoques filogenéticos para monitorear la dinámica de circulación de HAV, rastrear el origen de los brotes y respaldar medidas de salud pública.
