description Currently, prostate adenocarcinoma is considered a health problem of great relevance since it constitutes the second cause of morbidity and mortality among men over 50 in the Western Hemisphere. One of the methods for its early detection is to determine the serum values ​​of the prostate antigen. The tumor histological grade in these neoplasms constitutes a prognostic factor for therapeutic decision making.   The objective of this investigation was to determine the relationship between the Gleason Score and the values ​​of prostate specific antigen in biopsies of invasive acinar adenocarcinoma of the prostate. This is an observational, cross-sectional, retrospective and population-based study, where 78 cases of prostate biopsies obtained from the archives of the Service and 1st Pathology Department of the National Hospital of Clinics, of the city of Córdoba, were selected puncture between July 2017 and May 2019 with a diagnosis of invasive acinar adenocarcinoma of the prostate, which presented a complete clinical history performed in said hospital, together with a thorough physical examination. Of the 78 cases analyzed, 29 presented a diagnosis of invasive acinar adenocarcinoma of the prostate with a high Gleason Score of 8 or more. They were related to a specific prostate antigen value above 10 ng / dl (72%). Furthermore, it was observed that in those cases in which the antigen values ​​were high and had an adenocarcinoma with a Gleason score below 6 (67%), they were related to varying degrees of exacerbated chronic prostatitis. The realization of this work allows to establish that a high value of prostate specific antigen could be related to a high Gleason Score and therefore this determination would have predictive value of the prognosis in this variant of prostate carcinoma.

En la actualidad el adenocarcinoma de próstata es considerado un problema sanitario de gran relevancia ya que constituye la segunda causa de morbilidad y mortalidad entre los hombres mayores de 50 años en el hemisferio occidental. Uno de los métodos para su detección precoz es determinar los valores séricos del antígeno prostático.  El grado histológico tumoral en estas neoplasias  constituye un factor pronóstico para la toma de decisiones terapéuticas. El objetivo fue determinar la relación existente entre el Score de Gleason y los valores de antígeno prostático específico en biopsias de adenocarcinoma acinar invasor de próstata. Se trata de un estudio observacional, transversal, retrospectivo y de base poblacional, en donde se seleccionaron desde los archivos del Servicio y 1°Catedra de Anatomía Patológica del Hospital Nacional de Clínicas , de la ciudad de Córdoba, 78 casos de biopsias prostáticas obtenidas por punción entre julio del 2017 y mayo del 2019 con diagnóstico de adenocarcinoma acinar invasor de próstata, los cuales presentaban historia clínica completa realizada en dicho hospital, junto a un examen físico exhaustivo. De los 78 casos analizados, 29 presentaron el diagnóstico de adenocarcinoma acinar invasor de próstata con un Score de Gleason elevado de 8 o más. Los mismos se relacionaron con un valor de antígeno prostático específico por encima de 10 ng/dl (72%). Además se observó que en aquellos casos en los que los valores del antígeno estaban elevados y presentaban un adenocarcinoma con Score de Gleason por debajo de 6 (67%), tenían relación con grados variables de prostatitis crónica reagudizada. La realización de este trabajo permite establecer  que un valor elevado de antígeno prostatico específico podria estar relacionado con un Score de Gleason elevado y por lo tanto esta determinación tendria valor predictivo del pronóstico en esta variante de carcinoma prostático.
