description Within the non-surgical techniques of the saphenous magna the treatment with FOAM, is the practice of election today. This eco-guided sclerotherapy technique allows by endoluminal application of a detergent substance in the form of foam the denaturation of the endothelium, vasospasm and subsequent fibrosis of the vessel. The objective of this method is the treatment of the insufficiency of the saphenous magna, being the advantage of the method, the simplicity of its execution. It does not need to be performed inside an operating room, does not require any type of anesthesia and can be performed inside the clinic. This work was carried out in the service of Flebologia and Linfologia of the Hospital Nacional de Clinicas - FCM. Applied this treatment in the saphenous magna by foam in 21 patients, 13 females and 8 males, was used sclerosant idroxipolietoxidodecano to 1 % in the form of foam and the ultrasound scanner used was a Terason 3200 T. The technique used was 3 applications with a total 10 cm of foam in the region of saphenous magna. Permanent elastocompression for 96 hours. In the 80.9% of cases (17) was obtain a obliteration of saphenous magna after treatment, in the remaining 20.1% (4) required a second application to complete the result. Among the expected adverse reactions, pain and local phlogosis, were the most frequent and tolerable; phlebothrombosis occurred in 5% of cases (1), where local thrombectomia with good post-treatment evolution was required. The treatment with FOAM of saphenous magna is currently the technique of choice, for the good results obtained and for the possibility of performing it outpatiently. To date the results achieved in the service have allowed to solve the problem in the treatment of venous ulcers and tax venous gulfs of the saphenous magna in specifically selected patients. It is part of our project to continue with this methodology for which it has presented itself in the Training and Teaching Committee of the  Hospital Nacional de Clinicas for consideration.
