It was developed to ensure consistent and replicable tomographic reports. The diagnosis of infection is usually confirmed by polymerase chain reaction (RT-PCR), whose main disadvantages are the lack of availability and the delay in results. Objectives:-Categorizing, according to CO-RADS scores, tomographic findings in patients suspected of COVID-19.-Describing prevalent tomographic patterns in COVID-19.-Correlating the CO-RADS scale with the results of the RT-PCR test.  Materials and methods: A descriptive and retrospective study was carried out on 46 patients suspected of COVID-19 who underwent both chest CTs and RT-PCR tests, at the Hospital Italiano of Córdoba during October 2020.  All patients considered "suspicious cases" according to the Emergency Operations Center of the Province of Córdoba were included in the study. When CT scans were analyzed, a CO-RADS score was individually assigned and compared with the RT-PCR result. The research work was approved by the Research and Teaching Department of the Hospital Italiano of Córdoba. Results: CT scans were performed, on average, 4.85 days after the onset of the symptoms. The most frequent findings were bilateral and subpleural ground-glass opacities, present in 74% of the cases. When splitting patients into two groups, one composed of those with low suspicion of COVID-19 infection (CO-RADS 1 and 2) and the other composed of those with high suspicion of COVID-19 infection (CO-RADS 4 and 5), a statistically significant correlation (p=0.0332) between the CO-RADS scale and the RT-PCR result was found, with 25% positivity in the first group and 66% positivity in the second one. No CT with CO-RADS category 3 was found.  Conclusions: The CO-RADS scale may be a diagnostic tool for SARS-CoV-2 lung infection,  as it showed a correlation between categories 4-5 and positive RT-PCR results. Bilateral and peripheral ground-glass opacities are the most common findings. Further evaluations of the diagnostic performance of CO-RADS are needed.

Resumen:  Introducción: CO-RADS es un sistema de interpretación e informe estandarizado, creado por la Sociedad Radiológica Neerlandesa, que evalúa sospecha de neumonía por SARS-CoV-2 en tomografía computada (TC), en una escala de 1 (muy baja sospecha) a 5 (muy alta sospecha). Fue desarrollada para garantizar informes tomográficos uniformes y replicables. El diagnóstico de infección se confirma habitualmente mediante reacción en cadena de polimerasa (RT-PCR), cuyas desventajas principales son la falta de disponibilidad y la demora en los resultados. Objetivos:-Categorizar, según puntaje CO-RADS, hallazgos tomográficos en pacientes sospechosos de COVID-19.-Describir patrones tomográficos prevalentes en COVID-19.-Correlacionar escala CO-RADS con resultados de la prueba RT-PCR. Materiales y métodos: Se realizó estudio descriptivo, retrospectivo, de 46 pacientes sospechosos de COVID-19 del Hospital Italiano de Córdoba, a los cuales se les realizó TC de Tórax y RT-PCR, durante el mes de octubre del 2020. Se incluyeron aquellos pacientes considerados “caso sospechoso” según el Centro de Operaciones de Emergencia de la Provincia de Córdoba. Al analizar las TC, se asignó individualmente un puntaje CO-RADS y se comparó con el resultado de RT-PCR. El trabajo fue aprobado por el Departamento de Investigación y Docencia del Hospital Italiano de Córdoba. Resultados: Las TC fueron realizadas, en promedio, 4,85 días después del comienzo sintomático. Los hallazgos más frecuentes fueron opacidades en vidrio esmerilado, subpleurales y bilaterales, presentes en 74% de los casos. Al agrupar a los pacientes en dos grupos, uno compuesto por aquellos con baja sospecha de infección (CO-RADS 1 y 2) y otro compuesto por pacientes altamente sospechosos (CO-RADS 4 y 5), se demostró correlación estadísticamente significativa (p=0,0332) entre la escala CO-RADS y el resultado del PCR-RT, con un 25% de positividad en el primer grupo, y 66% de positivos en el segundo. No se encontraron TC con categoría CO-RADS 3. Conclusiones: La escala CO-RADS puede constituir una herramienta diagnóstica de infección pulmonar por SARS-CoV-2, ya que demostró correlación entre categorías 4-5 y un resultado positivo de RT-PCR. Las opacidades en vidrio deslustrado bilaterales y periféricos constituyen los hallazgos más frecuentes. Son necesarias más evaluaciones del rendimiento diagnóstico de CO-RADS. .
