The objectives were to characterize semen quality according to BMI in 20565 patients from the “Laboratorio de Andrología y Reproducción de Córdoba” (11/2006 to 12/2017); to evaluate their risk of suffering semen pathologies, and to establish one or more reference values in BMI scale, from which the risk of suffering sperm alterations is statistically elevated.
After signing the informed consent, patients’ BMI and semen quality were evaluated. Data were analyzed by ANOVA, Chi-square and Generalized Linear Models. Reference values were calculated by Multivariate Conglomerate Analyses K-means. Finally, a table with expected values of abnormalities for each new group resulted from BMI re-categorization was built.
Morbidly-obese and/or underweight patients showed the highest decrease in semen quality (in parameters like sperm concentration, total sperm count, motility and morphology; p<0,05) and higher chances of suffering semen abnormalities (oligozoospermia: 23,2% y 30,3% vs 14,6% in morbidly-obese, underweight and normal weight respectively; teratozoospermia: 47,5% y 55,4% vs 41,7% respectively y asthenozoospermia: 49,8% vs 34,7% respectively; p<0,05). The smallest summatory in sperm abnormalities were found at BMI=27kg/m2. Four reference points were identified, re-categorizing BMI in groups according to the risk of suffering semen disturbances. From lowest to highest: group1=BMI between 20 and 32kg/m2; group2=BMI<20 and BMI>32 to 37kg/m2; group3=BMI>37 to 42kg/m2 and group4=BMI>42kg/m2.
In conclusion, having a BMI below 20 or above 32kg/m2 is detrimental for semen; these negative effects increase from BMI>37kg/m2 and even further, from BMI>42kg/m2. The BMI re-categorization carried out in this study has andrological predictive power.

The BMI re-categorization carried out in this study has andrological predictive power. Frecuentemente se asocia al aumento del Índice de Masa Corporal (IMC) con disminución en la calidad espermática. Sin embargo, no se conoce a partir de qué valores de IMC estos efectos son significativos. Además, dado que la escala de IMC ha sido construida en base al riesgo cardiovascular, no necesariamente es aplicable a la función reproductiva. Los objetivos del trabajo fueros caracterizar la calidad espermática en relación al IMC de 20565 pacientes del Laboratorio de Andrología y Reproducción de Córdoba (reclutados entre 11/2006 y 12/2017); evaluar la asociación entre IMC y la frecuencia de patologías espermáticas y determinar la existencia de uno o más valores de corte en el IMC a partir de los cuales aumenta el riesgo de sufrir alteraciones espermáticas. Luego de la firma del consentimiento, se evaluó en los pacientes el IMC y se realizó un espermograma. Los datos se analizaron mediante ANAVA, Chi2 y Modelos Lineales Generalizados. Los valores de corte se calcularon con Análisis Multivariada por Conglomerado K-means. Finalmente, se realizó una tabla con frecuencias esperadas de anormalidades de acuerdo al IMC. Se detectaron alteraciones del espermograma en los pacientes con bajo peso o con obesidad mórbida (p<0,05), en los parámetros: concentración, recuento total, motilidad, morfología, alfa-glucosidasa neutra y fructosa. La incidencia de oligozoospermia y teratozoospermia, fue más elevada en estos dos grupos y la astenozoospermia fue más frecuente en los obesos mórbidos, en relación a los normopeso (oligozoospermia: 23,2% y 30,3% vs 14,6% respectivamente; teratozoospermia: 47,5% y 55,4% vs 41,7% respectivamente y astenozoospermia: 49,8% vs 34,7% respectivamente; p<0,05). El IMC en que se observó la menor frecuencia de alteraciones seminales fue de 27kg/m2. Se identificaron 4 valores de corte en el IMC, que dividían a los pacientes en 4 nuevos grupos con aumento progresivo y significativo en la frecuencia de anormalidades (p<0,05): 1) 20 a 32Kg/m2; 2) <20 ó >32 a 37Kg/m2; 3) >37 a 42Kg/m2 y 4) >42Kg/m2. En conclusión, un IMC <20 y >32Kg/m2 suponen un factor de riesgo para la calidad seminal.
