Moreover, vitamin D and its metabolites are known to be associated with multiple chronic diseases as diabetes mellitus, autoimmune, cardiovascular and neoplasia diseases. Objective: To assess the vitamin D status in a rural population of Córdoba Province in Argentina and its relation with bone mineral density. Material and Methods: We prospectively studied 31 patients over 50 years old who live in a rural population of Pampa de Achala in Córdoba Province, Argentina. This city is located in Córdoba High mountains. Blood vitamin D levels were tested in 24 patients and Bone mineral density in 31 patients. Vitamin D level was determined by HPLC. The vitamin D level were considered normal between 20-50 ng/ml in the winter season and 20-80 ng/ml in the summer season. Bone mineral density of lumbar spine and femoral neck was measured by DXA GE LUNAR DPX-L, according to World Health organization classification (37). The data were analyzed by Spearman coeficient and Chi cuadrado.Results: The vitamin D levels samples were avaliable in 24 patients. Mean blood Vitamin D level was 24.54 ng /ml. 8 of them (33%) had vitamin D level less than 20ng/ ml. 83% (20) of the analysed patients had vitamin D level less than 30 ng/ml and only 4 patients has vitamin D level more than 30 ng/ml.Low bone mineral density of lumbar spine and femoral neck was found in 66 % of the patients. 19 % of the patients were diagnosed of lumbar spine and femoral neck osteoporosis and 29% of them had lumbar spine meanwhile 25% had femoral neck. There was not statistically significant associaton between vitamin D level and bone mineral density analysed by Chi Cuadrado (p<0.07). A significant association was found between blood vitamin D level and low bone mineral density of femoral neck (Spearman 0.51) Conclusions: Vitamin D insufficiency is high in the rural adult population of Pampa de Achala in Córdoba and it could be a major health problem in this population.

Este hecho se asociada con cambios fisiológicos, patológicos, y evidencias clínicas, incluyendo incremento de la secreción de hormona paratiroidea, incremento del remodelado óseo, osteoporosis, leve osteomalacia y aumento del riesgo de fracturas de cadera. Además la investigación básica y varios estudios epidemiológicos sugieren que la vitamina D y sus metabolitos son importantes para la prevención de infecciones frecuentes y severas, diabetes, enfermedades autoinmunes, cardiovasculares y neoplasias. Objetivo: Conocer el nivel de vitamina D en una población rural de montaña en Córdoba y relacionarlo con la densidad mineral ósea medida por densitometría ósea.Material y Métodos: Se realizó determinación de vitamina D a 24 personas y densitometría ósea a 31 personas mayores de 50 años de ambos sexos que habitan en una población cerrada a 2.300 m de altura sobre el nivel del mar en Pampa de Achala, Altas Cumbres Cordobesas, Provincia de Córdoba, República Argentina, esto significó un 42% del universo. La Vitamina D se determinó por HPLC, siendo los valores de referencia en invierno: 20-50 ng/ml y en verano 20-80 ng/ml. La densitometría de columna y cadera fue realizada con equipo DXA GE LUNAR DPX-L, se usó la clasificación de la OMS de 1994 (37). La asociación entre las variables estudiadas fue analizada mediante el Coeficiente de Spearman y test de Chi cuadrado.Resultados: De 24 determinaciones de vitamina D, 8 de ellas (33% de la muestra) fueron menores a 20 ng / ml.; 20 fueron menores a 30 ng por ml, representando el 83% de la muestra. Solamente 4 pacientes tuvieron determinaciones mayores a 30 ng por ml. El promedio sérico de 25 hidroxivitamina D es de 24,54 ng / ml. Considerando la densitometría de dos regiones, columna y cadera, 66% de las personas estudiadas tienen baja densidad mineral ósea. En columna lumbar, 19% tienen osteoporosis y 29% con osteopenia, con predominio femenino. En caderas el 19%, tiene osteoporosis mientras que el 25% de la muestra presenta osteopenia. En el test de Chi cuadrado, si bien no se encuentra asociación significativa, existe una tendencia a confirmar con mayor número de casos, p<0.07; mientras que en la correlación de Spearman los valores de vitamina D sérica se asociaron a pobres valores densitométricos en mujeres en región trocantérica, con un valor de 0.51. Conclusión: En la población adulta de Pampa de Achala se constató elevado porcentaje de personas con déficit de vitamina D (83 %) lo cual condiciona negativamente a la salud ósea.
