A 56-year-old man, he was admitted to this institution due to deterioration of the general state and depression of the sensory system.Among its antecedents, we can mention: obesity, facial basocellular carcinoma, severe smoking (32p/y), vasculitis due to hypersensitivity with Ig A deposits (skin biopsy), very severe COPD (centroacinar emphysema and diffuse paraseptal), with functional dyspnea grade III - IV, OCD (chronic home oxygen therapy) since 1995, unilateral right lung transplant in 1999, with 13 days postoperative. Initial management with cyclosporine, azathioprine and meprednisone. Hombre de 56 años de edad, ingresa a esta institución por deterioro del estado general y depresión del sensorio.Entre sus antecedentes, se pueden mencionar: obesidad, carcinoma basocelular facial, tabaquismo severo (32p/y), vasculitis por hipersensibilidad con depósitos de Ig A (biopsia de piel), EPOC muy severo (enfisema centroacinar y paraseptal difuso), con disnea funcional grado III – IV, OCD (oxigenoterapia crónica domiciliaria) desde el año 1995, trasplante pulmonar unilateral derecho en el año 1999, con postoperatorio de 13 días.
