| Sumario: | Leishmaniasis are a group of diseases caused by infection with protozoa of the genus Leishmania, which parasitize the cells of the reticuloendothelial system. It is transmitted by the bite of female mosquitoes of the genera Phlebotomus and Lutzomya, whose reservoir are vertebrate animals. The disease is widely distributed in America and is endemic in the northern provinces of our country, where the agent involved is L. brazilensis. In America and Argentina there are three clinical forms: visceral (LV), cutaneous (LC) and mucocutaneous (LMC). Due to global warming, population migration and deforestation, the epidemiological border line has advanced towards our province, which forces us to keep this pathology in mind. The objective of the present work was to communicate two cases, uncommon in our environment, highlighting the importance of clinical-pathological correlation and morphological changes, sometimes subtle, which are observed in the biopsy. Its main importance is the early diagnosis and timely treatment, which prevents the progress of the lesions.
The first case is a 52 years old man, carpenter, native of the capital city of Córdoba. He had an erythematous plaque on the left cheek of two months of evolution, which began, after a trip to Jujuy, as an asymptomatic erythematous papule with progressive enlargement; currently covered with serohematic scabs, it measured 3-4cm in diameter and was surrounded by erythematous-edematous halo slightly indurated on palpation. The second case is a 76-year-old man, a native of the Punilla Valley, who consulted for a deep papular lesion in the calf, asymptomatic of 2 months of evolution after a trip to Brazil. In both cases, biopsies were obtained, fixed in 10% formalin, included in paraffin blocks and colored with hematoxylin / eosin, Giemsa, PAS and Ziehl Neelsen (ZN). In the histological sections were observed a dense inflammatory infiltrate, with plasma-histio-lymphocyte, visualizing the parasitic forms of extra and intracellular amastigostes.
Two cases of cutaneous leishmaniasis are reported, a rare condition in our environment, highlighting the importance of epidemiological antecedents to the correct diagnosis and timely treatment. |
